Patient / Individual Consultations

This is an opportunity for patients, family and friends of patients, as well as internet friends who would like counseling on their health and health issues. This consultation will establish what can be done to help assist you on your road to better health and the possibilities of further testing for diagnostic findings.

This phone consultation will be 1/2 hour in length. We work to accommodate requests, however, we have limited space each month.

Set up:

1. After purchasing your Consultation, return to this page and complete all forms and submit by mail or fax. Upon receipt of the paperwork, you will be contacted to schedule a consultation.

2. Print the following forms and complete:
